Guest Assumption of Risk, Release and Waiver of Liability
You, as guest, parent, or spouse, as applicable, hereby acknowledge that the use of Greate Bay Racquet & Fitness` facility, equipment, services, and or premises, do so at your own risk and assume all responsibility for such risk. You agree on behalf of yourself to release and discharge us, our affiliates, employees, agents, and representatives from all claims or causes on account of injury, loss, damage of property, arising out of our negligence. You acknowledge that you have carefully read this Waiver and fully understand that it is a release of all liability.

I authorize Greate Bay Racquet & Fitness to send text messages and/or telephone calls to my cell phone for any purpose, including marketing purposes concerning Greate Bay Racquet & Fitness. I agree that these calls or texts may be regarding the products and/or services that I have previously purchased and/or services that Greate Bay Racquet & Fitness may market to me. I understand that standard text messaging rates will apply to any messages received from Greate Bay Racquet & Fitness. I also understand that I or Greate Bay Racquet & Fitness may revoke this permission in writing at any time. I agree not to hold Greate Bay Racquet & Fitness liable for any electronic messaging charges or fees generated by this service. I further agree that in the event my cell phone number and/or cell provider changes I will inform Greate Bay Racquet & Fitness if I wish to continue to receive text messages and/or telephone calls. All members and guests consent for Greate Bay Racquet & Fitness to use the following: photograph(s) and/or electronic media images for presentation and promotional purposes.
