Member registration

• Payment due every 30 days after enrollment except for 1 year or 6 month membership.
• Membership is non refundable or transferable.
• Cancellation of membership must be done 30 days in advance in person. We may except cancellations via e-mail if unable to come in person. 
• Return EFT payments and or checks are subject to a $30.00 minimum fee 

I understand that participating in activities at God’s Gym involves risks, including the possibility of injury (physical or mental), death, or damage to my property. I acknowledge that:

• I may be injured or worse, and my property may be lost or damaged.
• Other participants’ actions may cause me harm or damage my property, and I may cause harm or damage to others.
• Conditions may change unexpectedly.
• God’s Gym, its staff, or agents may be negligent or in breach of contract, and there may not be adequate medical facilities or transportation if I am injured.

By participating, I accept full responsibility for any injury, death, or property damage that may occur.

I agree to release, indemnify, and hold harmless God’s Gym, its staff, and agents from any claims, actions, or liability of any kind arising from my participation, regardless of cause.

1. Each member is required to check in with membership key fob or mobile pass. One person per key fob or mobile pass. Door must be shut between every entry. Sharing is not allowed for membership and is subject to a $250.00 fine.
2. Do not allow anyone in gym unless they are your guest, (letting anyone in the gym will be a $250.00 fine ($10.00 guest pass).
3. Shoes and clothes must be free of dirt and debris prior to entering the facility. Member will be responsible for clean up of debris brought inside.
4. Disinfect equipment after use.
5. Re-rack all weights, dumbbells, and mats.
6. No pets allowed in gym.
7. No children allowed in gym.
8. No alcohol or drugs allowed on premises. If found to be intoxicated, you will be removed immediately with threat of being banned from facility. 
9. I am responsible for the cost of replacement or repair for equipment I damage.

I understand that my membership could be terminated if I don’t adhere to these terms and conditions.

EXPRESS WRITTEN CONSENT TO RECEIVE TELEMARKETING EMAILS, CALLS, and TEXTS: by signing this agreement, you (member) authorize the club/facility and/or its service providers, partners, affiliates, including, without limit to deliver, or cause to be delivered to member, at the telephone numbers or email provided by member in this agreement, telemarketing calls, telemarketing texts/emails, and similar communications using an automatic telephone dialing system or an artificial prerecorded voice. Member acknowledges that he/she is not being required to execute this consent (directly or indirectly) as a condition of purchasing any goods or services.
