REGISTRATION: Members must register at the front desk upon entering the club. Members may be required to produce their membership card and proper ID when registering.
PROPER ATTIRE: Proper gym attire must be worn when using the club. Appropriate athletic shoes and shirts must be worn.
ASSUMPTION OF RISK: Member understands the improper use of exercise equipment presents a serious risk of injury. Member assumes all liability and chooses to exercise with caution. Please report any potentially inoperable piece of equipment to a staff member, and do not proceed to operate or attempt to repair.
CLUB HOURS: OPEN Monday-Friday 4am-12am, Saturday-Sunday 5am-11pm.
CLUB RULES:
- Member agrees to comply with all the rules of conduct as set forth from time to time by the club management. Member understands that failure to comply with the club rules may result in termination of membership privileges.
-Member may cancel this agreement if University Fitness Center goes out of business and fails to provide facilities within (5) miles of or moves its location more than (5) miles from present facility. In such event, a prorated refund will be returned to the member. The refund shall be an amount computed by dividing the contract price by the number of weeks in the contract term and multiplying the result by the number of weeks remaining in the contract term. The business location of a health studio shall not be deemed out of business when temporarily closed for repair and renovation of the premises:
1. Upon sale, for not more than 14 consecutive days; or
2. During ownership, for not more than seven (7) consecutive days and not more than two (2) periods of seven (7) consecutive days in any calendar year.
- This membership agreement may be canceled if member dies or becomes physically unable to avail himself of a substantial portion of the services he used from the commencement of the contract to the time of disability. A Physicians written description of the member`s disability must be presented upon request of cancellation on these said grounds.
- The buyer should contact the Florida Department of Agriculture Consumer Services for information within 60 days should the health studio go out of business.
- The original membership term under this agreement shall not be for a period in excess of thirty six (36) months.
ELECTRONIC FUNDS (EFT) OR CREDIT CARD AUTHORIZATION - (AUTO-PAY ACCOUNTS ONLY) - THIS MEMBERSHIP WILL AUTOMATICALLY RENEW UNTIL 30 DAYS NOTICE IS GIVEN. MEMBER MAY CHOOSE TO CANCEL AT ANY TIME AND FOR ANY REASON.
I authorize UNIVERSITY FITNESS CENTER to automatically debit my account. If necessary, adjustments or entries to correct error are also authorized. I authorize my bank to honor preauthorized Electronic Funds Transfers (or credit/debit card charges) against my account for membership payments, WITHOUT OBTAINING ANOTHER PAYMENT AUTHORIZATION FROM ME.
SHOULD YOU (THE BUYER) CHOOSE TO PAY FOR MORE THAN ONE (1) MONTH OF THIS AGREEMENT IN ADVANCE, BE AWARE THAT YOU ARE PAYING FOR FUTURE SERVICES AND MAY BE RISKING LOSS OF YOUR MONEY IN THE EVENT THIS HEALTH STUDIO AND/OR THIS BUSINESS LOCATION CEASES TO OPERATE. THIS HEALTH STUDIO IS NOT REQUIRED BY LAW TO PROVIDE ANY SECURITY, AND THERE MAY NOT BE OTHER PROTECTIONS PROVIDED TO YOU SHOULD YOU CHOOSE TO PAY IN ADVANCE. UNIVERSITY FITNESS IS REGISTERED WITH THE STATE OF FLORIDA AS A HEALTH STUDIO. REG# H.S6323 THE MEMBER NAMED ABOVE REPRESENTS THAT HE/SHE HAS READ THIS AGREEMENT AND UNDERSTANDS IT`S CONTENT. THIS MEMBERSHIP IS NON-TRANSFERABLE AND NON-REFUNDABLE EXCEPT AS STATED ABOVE.
