Member registration

We understand that incidents happen from time to time or circumstances arise that may cause a member to need to cancel their membership. Below you will find the stipulations for canceling your American Muscle Factory (AMF) membership. - To cancel a membership the member must physically come in to American Muscle Factory to do so. Cancellations cannot be completed over the phone or e-mail. - The member must provide valid and current proof of ID. - There is a termination fee of $29.99 to cancel a contracted membership (‘No contract options’ can cancel at any time) - The member will also be charged half of the remaining balance on the contract. For example, if the member has 6 months remaining on their contract, they will be required to pay the cancellation fee of $29.99 plus 3 of the 6-remaining monthly dues) - The total amount due for cancellation must be paid at the time the member cancels. - If you signed up for a 12-month contract – it will auto-renew for another 12-months with the same terms unless the member cancels per the parameters above. I hereby acknowledge the above rules and requirements for canceling my AMF membership. I realize that if at a later time I would like to obtain another AMF membership it will require paying the initiation fee again along with any other associated fees or costs.

✓ WAIVER AND RELEASE OF LIABILITY: I, the undersigned, acknowledge and agree that participating in activities at American Muscle Factory (AMF) has inherent and assumed risks, including but not limited to Weight Lifting, Cardio Vascular Exercise, Personal Training, Therapy, and Power Lifting. Those risks may include: 1. Injuries due to the misuse of equipment, including but not limited to: i. Dropping dumbbells ii. Pinched or smashed fingers on machines iii. Improper form on exercises such as squats, deadlifts, etc. iv. Tripping or falling on/off the cardio vascular equipment 2. Muscular or tendon injuries from overexertion during physical exercise. 3. Injuries resulting from slipping, tripping, or falling inside AMF. I am aware of these inherent and assumed risks that could be present while training at American Muscle Factory. I ASSUME COMPLETE RESPONSIBILITY and liability for those risks and for the injuries that may occur as a result of those risks. I realize that by voluntarily assuming the risks involved, I will be SOLELY RESPONSIBLE for any loss or damage I sustain, including PERSONAL INJURIES to me or damage to my property. Representation of level of health and fitness. You represent to AMF that you are in good health and have no disability, impairment, injury, disease, or ailment that prevents you from engaging in active or passive exercise or which would cause an increased risk of injury or adverse health consequences as a result of such exercise. AMF will not provide medical treatment. You understand and acknowledge that neither AMF nor the personal trainers nor other AMF personnel have expertise in diagnosing, examining, or treating any medical condition. In the event you experience any illness, injury, discomfort, impairment, or other health problem (together referred to as a “Health Problem”) prior to or during a Training Session or your use of the Facility, you agree to (i) immediately inform AMF of such Health Problem, and (ii) consult your physician and reconfirm your physician’s consent to or approval of your continued participation in activities at AMF. In regards to personal property, we recommend that you not bring in any valuables into AMF. I understand the risk of taking personal belongings into a public place such as a gym, in that, those items could become lost or stolen. I understand that it is my responsibility to look after my own belongings and that AMF will not be held responsible if any items are lost or stolen. WARNING: This Agreement is legally binding. By signing it you give up your right to recover compensation through the courts or otherwise, for any personal injury, damage to, loss of, or stolen property. Which could be caused by negligence, misuse, accident, etc. that could occur at AMF. This Agreement shall be interpreted and construed in accordance with the laws of the State of Indiana. Should any action be instituted against AMF regarding the enforcement of the terms of this agreement, the prevailing party will be entitled to all of its expenses related to such litigation including, but not limited to, reasonable attorneys’ fees and costs, both before and after judgment. **I have read, understand, and agree to the above rules and conditions.**

All memberships (every account holder) will be charged a $30 annual renewal fee that comes out on the members billing cycle in January. Even if the account is on freeze, the account will still be billed in January for the annual fee. The fee is in place for equipment maintenance, new equipment upgrades, facility improvements, etc. 

American Muscle Factory (AMF) is Indiana's most distinguished and unique fitness facility. We provide a safe and encouraging environment for people to reach their fitness goals! At AMF we accommodate anyone and everyone from the average Joe looking to get in shape to the best athletes, bodybuilders or fitness enthusiasts around. We provide the highest quality equipment, personal training, and environment to help individuals reach their true potential! Rules At AMF we strive to create the most positive environment and one that supports all of our members. However, that doesn’t happen with just us. It takes effort from all of our members as well! We expect our members to follow the rules within the gym and those are as follows which includes but is not limited to: 1. Respect the gym i. Wipe down equipment after each use ii. Return things where you found them (attachments, dumbbells, smaller machines, etc.) iii. Clean up any messes you might have made (water or drink spills, chalk on the floor, etc.) iv. If you break something LET US KNOW v. Do not leave trash around the facility or on equipment vi. please do not poor water on the heating units in the saunas. vii. Keep locker rooms clean (place trash in appropriate receptacles) viii. If AMF offers towel service, be sure to place towels in appropriate towel bins when they are dirty. ix. Do not open the garage doors or any of the emergency exit doors 2. Respect fellow members i. Utilize gym etiquette ii. Please use proper gym hygiene (deodorant, washing hands, etc.). iii. Please wear proper gym attire. Athletic shoes, shorts or pants, shirts, tank tops, sports bras (with padding), are all fine. iv. No fighting or physical misconduct v. Do not make fun of any member for any reason, rather, lend a helping hand if someone is doing something wrong or let a staff member know so we may assist them vi. 3. Re-rack your weights i. Please return plates to their respective areas or weight trees ii. Please return Dumbbells to the dumbbell racks when finished with them iii. Please return specialty bars, benches, or other moveable pieces back to their respective holders or areas 4. Keep phone use to a minimum i. While on the gym floor, please keep phone use to a minimum. If you need to film your workout that is totally acceptable! Please try to avoid sitting or “camping” on machines while using your phone. This will allow members to move more quickly and efficiently in the space and keep their workouts going! If you need to make a call, email, etc., feel free to walk towards the locker room or lobby so you aren’t occupying equipment someone could be using. Hours of Operation American Muscle Factory is proudly operational 24 hours a day 7 days a week. Members may enter using their barcode at the door 24/7. However, we do have staffed ours and those are as follows: Staffed Hours: Monday - Thursday = 9am – 8pm Friday = 9am – 6pm Saturday = 10am – 4pm Sunday = 10am – 4pm By signing this form, you acknowledge that the gym will not always have staff members readily available. You understand the risk of using the facility during non-staffed hours and remove all liability from American Muscle Factory in the instance you are injured or an incident happens during non-staffed hours. You assume all the risk associated and understand this. Sneaking Non-Members Into the Facility American Muscle Factory is a 24-hour facility and has camera equipment to ensure safety in the gym at all times and so non-members are not entering when no staff member is present. The enforced policy for a member sneaking in a non-member after hours is as follows: 1st offense – Warning and the member will be charged 2x the day pass rate ($30). 2nd offense – Termination of membership. Age Ranges By signing this agreement, you are confirming you are over the age of 18 or that you are 16 or 17 years of age signing with a parent or legal guardian’s permission and signature. AMF does allow family member add-ons that can be from ages 10-18. Anyone ages 10-15 MUST be accompanied by an adult at all times while working out in the facility. Anyone ages 16-17 can use the facility without a parent or guardian present but must come ONLY during staffed business hours. Breaking or Freezing Contract By signing up for one of the memberships that has a 12-month contract associated, you understand that you are obligated to pay 12 months’ worth of membership dues over the course of said membership. Failure to do so will result in legal action taken by American Muscle Factory. The contract will auto-renew for another 12-months with the same terms unless the member cancels. By signing this agreement, you understand that a collection will be filed if a membership contract is broken and a balance remains unpaid. AMF also offers a Freeze Account option. The freeze account policy is as follows: - $29.99 freeze fee - Member can keep the account on freeze for up to 12 months Personal Training Agreement AMF offers high end personal training. Please ask your AMF staff member about which personal training option is right for you! Longevity Athletic Training AMF is proud to offer athletic training services / massage therapy and rehab within our facility. Please ask an AMF staff member for more information on these services. Zero Tolerance Policies At AMF we look to facilitate the most motivating and welcoming environment to all of our members. We want our members to respect one another and our facility and help us make it the best gym in Indy! We ask that you read and understand these zero tolerance policies which include but are not limited to: - Filming someone without permission - Sexual harassment or inappropriate comments - Sexual misconduct within the facility - Fighting or any physical confrontation - Excessively throwing dumbbells, plates, machinery, etc. - Bullying, name calling, etc. Agreement Statement **I confirm that I have read and understand the Membership Agreement for American Muscle Factory. I confirm that I am 18 years or older and signing this form of sound body and mind. AMF is entitled to revise this Agreement at any time and may notify me of the changes and updated Agreement by electronic mail to the e-mail address on file**
