Member registration

**Month-to-Month Memberships:** The member identified above authorizes Gym Bobs Fitness Center to process a one-time payment for the total membership fee at the time of signup. Once the membership period concludes, the contract will be terminated, and access to the facility will be revoked unless the member has provided alternative notification to Gym Bobs Fitness Center.

**Automatic Withdrawal Memberships:** The member identified above grants Gym Bobs Fitness Center permission to automatically deduct monthly dues from their bank account or credit card. Payments will continue to be withdrawn on a monthly basis until the membership is canceled. The member acknowledges that if they are the primary account holder, they are responsible for all payments associated with any additional members listed on the contract.

By signing this contract, the member identified above acknowledges that failure to utilize the membership does not absolve them of their payment obligations, irrespective of the circumstances. Upon completion of the initial membership term, the contract will transition to a month-to-month basis, with the member billed at the same rate as the initial membership. It is the member`s responsibility to cancel the membership. To do so, the member must provide a five-day notice to Gym Bobs Fitness Center via email at [email protected], by calling (615-598-6724), or by directly contacting a staff member at the center.

If the member opts to use a bank account or debit card for payment, a $15 fee will be applied for each instance of rejected payment. The member is also responsible for keeping their billing information up to date. The member agrees not to share their assigned key fob or any other member`s key fob for facility access. Additionally, the member must not permit non-members to enter the facility. Should a member provide a key fob to a non-member or grant them access, Gym Bobs Fitness Center reserves the right to terminate the contract immediately without a refund and charge the member who permitted access a fee of $10 per day. By signing this contract, the member agrees to all terms outlined herein.

  * Members or guests between the ages 16 and 18 years must have a signed liability waiver from a parent or guardian to use the equipment and/or tanning beds at Gym Bobs Fitness Center.

   * Members or guests between the ages 13 and 15 years must have a signed liability waiver from a parent or guardian and must be accompanied by an adult at all times. Children under the age of 16 are prohibited from using the tanning beds. 

   * Member understands that as a member of Gym Bobs Fitness Center he/she will not allow children age 0-12 in the building. 

   * Member understands that he/she shall not give anyone else their assigned key fob or another members key fob to access the facility. Member understand that he/she shall not allow non-members access into the facility. Tanning members understand they cannot allow anyone access to the tanning area that does not have a key fob. Tanning only members agree they will not be allowed inside the gym area or use any gym equipment.  In the event a member gives out a key fob to a non-member or allows non-members access to the facility, Gym Bobs Fitness Center has the right to terminate the contract immediately and require remaining amount of monthly dues, on the initial contract, to be paid within 30 days and remove access to the facility immediately. If the contract is a paid-in-full membership, Gym Bobs Fitness Center has the right to terminate this contract early, remove access to the facility and will not return any dues/fees that were paid.

   * Member understands they are responsible for any guest(s) they bring into the facility. Member understands they are responsible for any injuries or damages caused by the guest to the facility, to another member or non-member. Guest(s) without a contract are not permitted to use the facility unless guest fee of $10 is paid. Guest(s) must be accompanied by the member at all times.

   * The member understands and acknowledges the dangers that can be present in using the equipment at Gym Bobs Fitness Center facility. By signing this contract, the member hereby releases Gym Bobs Fitness Center of any and all responsibility and liability that accompanies any accidents, expenses, or other occurrences within and on the property of the facility. Gym Bobs Fitness Center is not liable for any injuries or damages to any member(s), non-member(s) or guest(s), or the property of any member(s), non-member(s) or guest(s), or will be subject to any claim, demand, injury or damages whatsoever, including without any limitations, those damages resulting from acts of active or passive negligence on the part of Gym Bobs Fitness Center. 

   * Member understands that he/she is responsible for any damages to the facilities, equipment, property, and for any personal injury or property damage caused by the member to any other member(s), non-member(s) or guest(s).

Members between the ages of 16 to 18 years old must have a signed liability wavier from a parent or guardian to use tanning beds at Gymbobs Fitness Center. Children under the age of 16 are prohibited from using the tanning beds. 

Tanning Bed Warning Statement
• Failure to use the required eye protection provided to the customer may result in damage to the eyes.
• Overexposure to ultraviolet light may cause severe sunburn reactions;
• Repeated exposure to ultraviolet light may result in skin cancer and premature aging of the skin.
• Abnormal skin sensitivity or severe burning may be caused by reactions when tanning devices are used in conjunction with certain foods, cosmetics, and medications such as tranquilizers, diuretics (fluid pills), antibiotics, high blood pressure medicines, and birth control pills.
• An individual taking prescription medication or over-the-counter drugs should consult a physician before using a tanning device.
• An individual with skin that tends to burn easily, freckles, or never tans should avoid use of a tanning device before consulting a physician.
• An individual with a family or personal past medical history of skin cancer should avoid use of a tanning device.
• An individual should allow a minimum of twenty-four (24) hours between uses of a tanning device.
• Pregnant women or women using oral contraceptives may develop discolored skin due to the use of tanning devices.

By signing this contract, the member, listed above, understands that Failure of the member(s) to use the membership does not relieve the member of their liability for payment, regardless of circumstances. Once the duration of the initial membership has passed, the contract will automatically roll to month-to-month. Member will be billed the same amount as the initial membership. Member is responsible for canceling membership. To cancel membership, member must notify Gym Bobs Fitness Center, with a 5 day notice, via email to [email protected], by phone number (615-598-6724) or directly contacting Gym Bobs Fitness Center staff member. If the member, stated above, chooses to use a bank account or debit card for membership dues, they will be charged a $15 reject fee per occurrence. Member will be responsible for updating billing information. Member understands that he/she shall not give anyone else their assigned key fob or another members key fob to access the facility. Member understand that he/she shall not allow non-members access into the facility. In the event a member gives out a key fob to a non-member or allows non-members access to the facility, Gym Bobs Fitness Center has the right to terminate the contract immediately with no refund and charge the member who allowed access their $10 day fee. By signing, member agrees to these terms.
