Member registration

Your membership will continue to draft at the rate after the billing cycle as listed above, regardless of use or participation until proper notification is given to the club. I hereby accept the option of a continued membership on a monthly basis. All Memberships will require a 30-day cancellation notice.

ALL CANCELLATIONS MUST BE MADE IN THE CLUB BY FILLING OUT A CANCELLATION FORM.

$29 Early Cancelation Fee if canceled within first 6 months.

YOU, THE BUYER, MAY CANCEL THIS MEMBERSHIP AT ANY TIME PRIOR TO MIDNIGHT OF THE THIRD BUSINESS DAY AFTER THE CONTRACT DATE SHOWN BELOW. TO CANCEL, YOU MUST NOTIFY THE CLUB, IN WRITING, OR IN PERSON.

The Club urges you and all members to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise class. All exercises, including the use of weights and use of any and all machinery, equipment, and apparatus designed for exercising shall be at the member's sole risk. Member understands that the agreement to use, or selection of exercise programs, methods and types of equipment shall be member's entire responsibility, and the Club shall not be liable to member for any claims, demands, injuries, damages, or actions arising due to injury to member's person or property arising out of or in connection with the use by member of the services, facilities, and premises of the Club. Member hereby holds the Club, its officers, owners, agents and employees harmless from all claims which may be brought against them by member or on member's behalf for any such injuries or claims. Member agrees to follow club rules as promulgated. Violation of these rules may be the cause for suspension or cancellation of membership.

Fuel Fitness Enhancement Fee is a $25 annual fee that is scheduled 60 days from your membership start date. This fee will be deducted every year on the same date for the duration of your membership.

This agreement represents the complete understanding between the Member and Lifestyle Wellness & Spa. No representations, written or oral other than those contained within this agreement are authorized by or binding upon Lifestyle Wellness & Spa

I understand that I have signed hereunder a contract/note which will be processed by Lifestyle Wellness & Spa using our provider Health Club Systems. My failure to regularly utilize LSW facility does not relieve me of my obligation, regardless of the circumstance, to pay all agreed monthly payments.
