Member registration

CANCELLATION: There is no cancellation fee for Month-to-Month EFT memberships, but you must give a minimum of 10 DAYS NOTICE prior to the next billing date. No refund will be granted for dues incurred before Pavitt Health & Fitness was notified.
If by reason of death or permanent disability, the buyer is unable to continue the membership, buyer or buyer’s estate shall be relieved from the obligation of this contract. No refund will be granted for dues incurred before Pavitt Health & Fitness was notified of the disability or death.
EFT: EFT (electronic fund transfer) contracts are open-ended. Funds will continue to be transferred on a monthly basis until a cancellation request is submitted. Until you cancel your membership, you are responsible for all payments, regardless of gym usage. A $20.00 decline fee will be added to each month that we are unable to receive funds via EFT. If a balance accumulates on your account it will attempt to pull the entire balance on the next monthly charge. Should you default on any payment obligation as called for in this agreement, the entire remaining balance shall be deemed due and payable upon demand, and you agree to pay allowable interest, and all costs of collection, including, but not limited to, collection agency fees, court costs, and attorneys’ fees.

The fitness center urges all members to obtain a physical examination from a doctor before using any exercise equipment, participating in any exercise classes, or working with a personal trainer. All exercises, including the use of weights and use of any and all machinery, equipment, and apparatus designed for exercising shall be at the member’s sole risk. Member understands that the agreement to use, or selection of exercise programs, methods, and types of equipment shall be the member’s entire responsibility, and that Pavitt Health and Fitness shall not be liable to member for any claims, demands, injuries, damages, or actions arising due to injury to member’s person or property arising out of or in connection with the use by member of the services, facilities, and premises of Pavitt Health and Fitness. Member hereby holds Pavitt Health and Fitness, its officers, owners, agents, and employees harmless from all claims which may be brought against them by member on member’s behalf for any such injuries or claims.

NON-DISCRIMINATION POLICY: We do not tolerate discrimination or inappropriate behavior towards any member based on their race, ethnicity, nationality, gender identity, ability, family structure, sexual orientation, body size, fitness level, or for any other reason. Employees and members will use the locker room that coincides with their gender identity. Inappropriate behavior will result in the immediate cancellation of your gym membership and if appropriate, the police will be notified.
GENERAL:
Use of Pavitt Health and Fitness Center is for current members only.
Allowing non-authorized persons to use your key or to train with you will be prosecuted as theft of services.
No drugs or alcohol are allowed on the premises. Members may not train or be on the premises while under the influence of drugs or alcohol. Members agree not to use anabolic steroids.
You must re-rack your weights and return items used to their appropriate locations. Please wipe down all equipment after use.


WET ROOM: The wet room is only open during front desk hours. Members must shower off before using the hot tub or cold plunge. You must also shower off when going from the sauna into the hot tub or cold plunge. ABSOLUTELY NO water can be put on the sauna heater or towels hung above the sauna heater or on the thermostat. CHILDREN: Children under 10 years of age are not allowed on any equipment at any time. Children ages 10-14 may train only under direct adult supervision and must also be directly supervised while in the wet room. On their 19th birthday, children will no longer qualify to be joined with their parents on a family membership. If they choose to keep their membership, they will need to sign a new contract as an individual.
