Policy Statement
1. Membership Eligibility

Membership is open to individuals 18 years or older.

Individuals under 18 must have a parent or guardian sign the membership agreement.

Children under 10 years old are not allowed in the Fitness Center.

Youth aged 10–12 must be accompanied by an adult (18 years or older) while using the Fitness Center.

Teens aged 13–17 can use the Fitness Center independently after completing a mandatory orientation session with a staff member.

All members must complete and sign the membership agreement and waiver before using the facility.
2. Membership Types

Individual Membership: Access to all gym amenities.
-
Couple Membership: Members can add a roommate, partner, spouse, or child to their membership. This membership is limited to two individuals per account.

Family Membership: Restricted to immediate family members only (no in-laws) living under the same address. Family memberships can include up to five members. Verification of address is required. If verification is not provided, the primary account holder will be charged the individual membership rate.

Short-Term Memberships: Day passes and week passes are available for guests.
3. Payment and Billing

Membership dues are billed monthly, with payments due on the 1st of each month.

An annual fee of $49 will be charged to all active memberships.

Membership billing will continue until canceled by the member using the method outlined in this agreement.

A $3.00 transaction fee will be added monthly to payments made via credit card or debit card.

A $25 service fee will be charged for any uncollectible transactions.

Changes in billing account information must be submitted in writing or updated at the front desk at least 10 days before the billing date to avoid service fees.
4. Lack of Use Policy

Lack of use does not relieve the member of past, present, or future monetary obligations.

Refunds will not be issued for lack of use.
5. Membership Freeze Policy

Frozen memberships will incur a minimum monthly fee of $8.00.

Frozen memberships cannot be canceled during the freeze period.

Frozen memberships are for an initial term of 2 months and will revert to the full membership rate after the frozen term concludes unless extended with documentation.
6. Membership Cancellation Policy

Memberships are month-to-month and can be canceled at any time with a 30-day written notice.

Members may cancel their membership by providing written notice in person, via email, phone call, or text.

For all cancellations requested via email, phone call, or text, members will receive an email confirmation and must complete the cancellation process from their side.

Members may also cancel their membership by providing written notice, postmarked by certified or registered mail, within three (3) business days of signing the membership agreement to the address specified in the contract.

Proper athletic attire and footwear are required at all times.

Members must wipe down equipment after use with the provided sanitizing wipes.

No personal training is allowed without prior approval from management.

Lockers are for daily use only. Use lockers at your own risk, and we recommend locking them before leaving any valuables.

Basketball Court Use: Members may use the basketball court when it is not scheduled for other activities. Members must follow the basketball court rules and regulations in order to use it.

Sauna Policy: The sauna is a dry sauna; members must not use water or tamper with the sensor and controller. Personal belongings should not be left inside the sauna. Members should consult their doctor before use if they have any health conditions. Use of the sauna is at the member`s own risk.
11. Code of Conduct

Quincy Athletic Club reserves the right to refuse service to anyone for behavior deemed inappropriate, disruptive, or unsafe by management.

Members are expected to treat staff, equipment, and other members with respect.

Harassment, discrimination, or aggressive behavior will result in immediate membership termination without refund.

Breaking posted club rules or regulations will result in membership termination at management’s discretion.

Lack of use does not relieve member of past, present, or future monetary obligations. Refunds will not be given for lack of use. Memberships cannot be frozen unless recommended or accompanied by a certified DR`s note and/or deployed on military leave. Frozen memberships cannot be cancelled. Frozen memberships will be charged a minimum monthly fee of $8.00. Frozen memberships are not applied against the 12 month minimum contractual term. Unless accompanied by a DR`s note and/or deployed on military leave - Frozen memberships will be for an initial term of 2 months and will automatically revert to full membership monthly rate after frozen term has concluded. Monthly dues are subject to annual increases after the initial twelve (12)month contractual period has concluded. Accounts 60 days past due will be terminated and sent to a third party collection agency. It is further understood that breaking any of the posted club rules and regulations will result in membership termination at he discretion of management. A $25 service fee will be charged for all transactions returned uncollectable. Changes in billing account information must be received in writing at least 10 days prior to billing date in order to prevent service fees. A $3.00 transaction fee will be charged monthly and added to the monthly payment for all memberships billed via credit card and/or debit card. A $10 fee per offense will automatically be charged for any person registered for a class who no shows without canceling within 4 hours of class start time. Membership billing will continue until member cancels monthly payment by method outlined in this agreement. Customers right of cancellation: You may cancel this contract without penalty or further obligation by causing a written notice of your cancellation to be delivered in person or postmarked by certified or registered United States Mail within three (3) business days of the date of this contract or the date of your receipt to the address specified in the contract.
In consideration of NE Health and Fitness DBA Northeast Health & Fitness allowing me to be a member and permitting me to use its facilities and equipment, I agree to the following: In attending the Northeast Health & Fitness Club(s) and using its facilities and equipment, I agree that I am doing so at my own risk. The Northeast Health & Fitness Club(s) shall not be liable for any damages arising from personal injuries sustained by me on the premises. I assume full responsibility for any injuries which may occur to me on the premises of the club(s) and release and discharge Northeast Health & Fitness Club(s), its owners, its employees and its agents from any and all claims resulting from my use of the facilities and equipment of the club. I represent that I am in good physical condition and that I have no disability or impairment preventing me from engaging in active or passive exercise.
