EFT Auto Renew Notice: Provided Membership is in good standing, membership will automatically renew at the rate indicated above until canceled by member or EFC. *Cancellation of your membership requires the filling out of a Membership Cancellation Form in person during staffed hours on or before the 25th of the month prior to the month in which you wish to cancel in order to avoid further charges. Dues must be current at time of cancellation. In special approved circumstances, we may accept cancellations via e-mail if unable to come in person. Early cancellation of any contract is subject to a cancellation fee of not less than 3 months of membership dues or a minimum of $200, whichever is greater and is subject to approval by management. (other exclusions apply)
Member Assumption of Risk and Release:
Member Assumption of Risk and Release: I understand the risk of injury from club activities and using any club equipment is significant, including the potential for permanent paralysis and death. I knowingly and willingly accept all such risk, both known and unknown. I acknowledge that this may be an unsupervised club at times and I assume all risks associated with using exercise equipment and exercising along without the aid and presence of club staff on the premises. I expressly agree that this release is intended to be as broad and inclusive as permitted by applicable law and if a portion of this release is held invalid, the balance shall remain in full force and effect. This release shall apply to my heirs, assigns, personal representatives and any other next of kin. I understand the club is relying on this release in agreeing to enter into this agreement. In consideration of the right to use the facility and premises, I agree to defend, indemnify and hold Emerald Fitness Club, LLC and its officers, agents and employees harmless from and against any claim for injury or damage to myself, any listed minor children or others arisen from use of the club for any purpose. Furthermore I have read the rules and regulations set forth by Emerald Fitness Club and hereby agree to follow them. If not followed my membership may be revoked immediately. I agree that my membership may be canceled without cause at anytime. If you believe there is a risk to your health by participating in a fitness service at this fitness center, you must inform the supplier in writing about the risk. If you are a casual client, you must assess your own fitness level, ability to exercise, and risk to your health by participating in a fitness service at this fitness center and utilize facility according to your assessment. If you utilize the facility during non staffed hours, you understand that you are working out at your own risk. Members are required to know where all emergency exits and emergency call buttons are. If you are using the facility alone, you further agree to wear an emergency device provided by EFC.
Annual Maintenance Fee: Monthly dues from members helps pay for the day to day operations of EFC. The purpose of the annual maintenance fee is to repair and or replace equipment throughout the year. This annual fee of $30 will be drafted annually on March 1st.

Here at Emerald Fitness Club we are committed to your safety, to your fitness education, to the cleanliness of the facility and to the overall positive daily experience when you walk through the front door.
*Each Member is required to check in with membership card upon entering the facility. One person per key card. Sharing is not allowed for membership and is subject to a $250 fine.
*Equipment utilized on the gym floor must be returned to proper location upon completion of use! Bars must be unloaded, and plates returned to appropriate racks.
*Covered toed shoes, shirts, and appropriate workout attire are required at all times. Shoes and clothes must be free of dirt and debris prior to entering the facility. Member will be responsible for clean up of any debris brought inside.
*We like to see you sweat, just not all over our equipment.. Please bring and utilize a sweat towel if you find yourself profusely sweating as sweat all over our equipment is not desired by anyone. Please prevent sweat from falling onto floors and equipment and clean up after yourself.
*Please wipe down the equipment you use when finished. Paper towels along with sanitary spray bottles are conveniently located throughout the facility for this purpose. Please utilize them!
*You are buying a gym membership and are agreeing to pay dues according to the plan chosen regardless of the frequency of gym use.
*Your membership is non-refundable and non-transferable.
*Any and all pre-paid memberships are non refundable. If the gym is closed temporarily for more than 30 days, EFC will extend the pre-paid expiration by the same amount of time as closed. Other exclusions may apply.

*Returned EFT payments and or Checks are subject to a $30 minimum fee.
*Emerald Fitness Club is not responsible for lost or stolen items. Personal belongings are not to be stored in the day use lockers overnight. Locks are not provided.
*No Children under the age of 10 will be allowed on the weight or cardio floor. Children ages 10-14 require parental supervision at all times while on the weight floor.
*Faulty or broken equipment should be brought to the attention of the staff immediately.
*No Alcohol or drugs allowed on premises. If found to be intoxicated, you will be removed immediately with threat of being banned from facility.
*Guest of members are allowed! HOWEVER…ANY and ALL guest brought to the gym, are REQUIRED to sign in with staff and or fill out an envelope (located at the front entrance) and pay the guest pass fee. 1st FAILURE to have your guest sign in and pay WILL RESULT IN AN AUTOMATIC $25 draft to your account for each person let in. 2nd FAILURE will result in $250 draft to your account for each person let in and membership expulsion.
*24/7 Access:
Locate all emergency exits and 911 call buttons in case of emergency. (WHEN PRESSED, EMERGENCY PERSONNEL WILL BE DISPATCHED TO CG LOCATION IMMEDIATELY) If you are using the facility and are alone you are required to wear an EMERGENCY LANYARD provided by EFC in case of emergency. For your safety, members are NOT allowed to open the door for anyone. Every user must have their own active key card. Allowing anyone other than yourself into the facility without signing guest pass will not be tolerated.
