Member registration

Monthly membership fee is $30 plus applicable tax for the first member and an additional $10/month plus applicable tax per family member on account. Membership will be billed on the 15th of every month and continues until membership is officially canceled.

I authorize Unity Fitness Center to electronically deduct (EFT) my monthly dues or any past dues from the account listed above. The deductions may begin on the above date and continue until my membership is terminated, canceled, or I tell Unity Fitness Center in writing to stop deductions, which may take up to 60 days from the time notice is given until payments/deductions are canceled. Assumption of Risk: the use of the facility at Unity Fitness Center naturally involves the risk of injury to you or your guest, whether you or someone else cause it. As such, you understand and voluntarily accept this risk and agree that Unity Fitness Center will not be liable for any injury including, without limitation, personal, bodily, or mental injury, economic loss or any damage to you or your spouse, guests, unborn child, or relatives resulting from the negligence or other acts of Unity Fitness Center or its associates. This agreement is not effective until you and an authorized Unity Fitness Center representative sign and date it. By signing it below, you agree to all the terms of the agreement.

Annual fee billing of $29 plus applicable tax per person, per year for all members of Unity Fitness. This annual fee will be billed on July 3rd of each consecutive year to the account information supplied for monthly dues.

Monthly memberships require 60-day written notice to stop billing. All cancellations must be done in person at Unity Fitness Center or through certified mail. A prepaid membership is non-cancelable and the prepaid monthly enrollment is non-refundable. If you violate the gym policy it will result in termination. Under no circumstances are you allowed to let anyone in the gym. Terms,fees,and conditions are subject to change.

There will be $5 charge per offense of not racking weights.
