In consideration of being allowed to participate in any way in the program, related events, and activities, I the undersigned, acknowledge, appreciate, and agree that:
1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis from my participation.
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation.
3. I willingly agree to comply with terms and conditions for participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.
4. I, for myself and on behalf of my heirs, assignee, personal representatives and next of kin, HEREBY, RELEASE, INDEMNIFY, AND HOLD HARMLESS THE Allen Samuels Sports, Inc; DBA Waco Regional Tennis & Fitness Center, its officers. Officials, agents and/or employees, other participants, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (RELEASEES), from any INJURY, DISABILITY or DEATH, I may suffer, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT; FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

This is to certify that I, as a parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above all the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liability incidents to my minor child’s involvement or participation in these programs as provided above, Even IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law.

As a member of the Waco Regional Tennis & Fitness Center, I will comply with and be bound by the rules of the Club or Center as they may be amended from time to time. I understand that participation in any athletic activity may cause injury and may possess health risks. I waive all legal claims against the City of Waco, Allen Samuels Sports, Inc., its employees and agents from any injury or damage that I, or any member of my family, might incur during or as a result of participation in an activity sponsored by the Club or Center.

annual fee terms

Fitness Membership Terms:
• Must be 16 years of age to use facility without a parent or guardian. Children under the age of 16 must be with an adult.
• Family agreements include: 2 heads of the household, and any children ages 15-21. Children older than 21 will need to have their own agreements.
• Couple Memberships include any two family members under the same household.
• 15 years of age is the minimum age requirement to use the fitness center or take any workout classes.

Fitness Month-to-Month Memberships:
• Recurring Billing Cycle: Initial prorated payments occur at the time of registration and recurring charges debit on the 5th of each month, until a 30-day notice of membership cancellation is processed.
• 30-day cancellation policy: A 30-day emailed or written notice is required to cancel or terminate any and all memberships. Cancellation will not be prorated. Late notices will result in continued billing until cancellation terms are met.
• No refund policy: Once a member registers and pays for this membership tier, there are no refunds under any circumstances.
• Subject to change: WRTF reserves the right to make any and all changes to class times, schedules, and terms and conditions without any notice.

Annual Memberships:
• All annual Memberships are paid in full at the time of purchase.
• Annual Memberships are nonrefundable.
